Everyone deserves a chance at recovery, especially those in our community's most vulnerable populations.
This is the core belief for the Adult Transition Team (ATT) as they help and support individuals who have recently been released from incarceration or are on the verge of becoming homeless as they move through recovery.
This full-service partnership team provides intensive treatment, services, and support to individuals with a serious mental health and/or substance use disorder. Those in the program work with staff to build their personalized treatment plans and determine which supports are needed to learn how to manage their symptoms and rebuild their lives.
ATT aims to reduce homelessness, incarcerations, and hospitalizations, while increasing education and employment.
Criteria
Participation in the program is completely voluntary. Referrals must come from attorneys, state and inpatient hospitals, probation, parole, CDCR and Lerdo's jail facility. To qualify for services, a person:
- Has been diagnosed with a chronic mental health condition that severely impacts their daily functioning
- Is homeless or at risk of homelessness
- Must be currently involved in the legal system
Services
- Intensive case management services
- Crisis counseling
- Psychiatric services
- Psycho educational groups focusing on anger management, coping skills, grief loss and relapse prevention
- Substance abuse counseling including: 12-step
- After hours phone support
- Housing assistance/placements
- Linkage to employment and education
- Job development and coaching
- Connection to community resources
- Linkage to Narcotics Anonymous/Alcoholics Anonymous meetings.
